New Project Benefits South Island Seniors With Chronic Disease

VICTORIA – The first phase of the Vancouver Island Health Authority’s (VIHA) new Seniors-At-Risk integrated network project will improve access and coordination of care at the community level to seniors suffering from chronic disease.  As part of the province’s Health Innovation Fund, this initiative received $1.2 million in funding last year, and includes participation from 15 physicians in the Greater Victoria and Saanich Peninsula areas.  Through this community-focused network, this initiative is expected to benefit as many as 1,400 South Island seniors this year.  Read More (PDF)
