Advance Directives

Helping people make end-of-life, emergency, and critical health care decisions in advance.

Have you considered the kind and level of medical care you would want if you fell ill or were injured and were unable to express your wishes directly?

The materials associated with VIHA’s advance health care planning program have been developed for use within the Vancouver Island Health Authority. Therefore:

  • if you live elsewhere in British Columbia, consult your health authority as it may be introducing another form of advance directive;
  • if you live in other provinces in Canada or in the United States, please consult your local legal information agencies or government website as the VIHA advance directive will not be consistent with laws governing the use of these documents.

SPECIAL NOTE: Supplies of VIHA’s Let Me Decide advance health care planning forms and informational material are currently out of stock and will not be renewed.

VIHA has prepared an interim advance health care planning information package and form that you can download from this link:
