GROUP BENEFITS: FORMS

Print, complete and return the forms to Employee Records & Benefits - #300 - 6475 Metral Drive, Nanaimo, BC V9T 2L9.

Please DO NOT return benefits forms directly to the Medical Services Plan (MSP) or Pacific Blue Cross (PBC).

Regular FULL-TIME Union Employees

The following 2 forms, if required, must be completed and returned directly to the Municipal Pension Plan address on the form.
