Convalescent Care is short-term care for medically stable clients who no longer need acute care in hospital, but do need more time to recover before returning home.
In VIHA, Convalescent Care is available through the Activation Program. This program provides comprehensive rehabilitation services to clients in transition between acute care and home or a different home environment. Activation Programs are available in:
- Aberdeen Hospital, Victoria (30-bed unit)
- Dufferin Place, Nanaimo (30-bed unit)
- Cairnsmore Place, Duncan (20-bed unit).
The length of stay depends on your needs. During this time, your care team will help you regain strength and mobility so you can safely carry out your activities of daily living when you return home. The health professionals in your care team may include nurses, occupational therapists, physiotherapists, personal care workers and others as needed.
If you are in the hospital, access to the Activation Program is through the Hospital Liaison Case Manager covering your unit. If you are at home, contact your Case Manager.
How much does it cost?
Convalescent care rates are set by the provincial government at the lowest residential care rate. For more information click here.
