New Patients
If you are new to Victoria, are 55 years of age and over and do not have a family doctor, or if your doctor is retiring, we might be the primary health care centre for you.
Please note that we are not able to accommodate patients who currently have a family doctor.
It is also important to note that the geographic boundaries for Health Point Care Centre are south of McKenzie Avenue and east of Douglas Street. This means that new patients to our practice must live within these boundaries. View the Geographic Boundary Map (pdf). All potential new patients are asked to complete a confidential health questionnaire. Once the questionnaire is reviewed, and if approved (you must meet the age, boundary and no family doctor criteria), our staff will contact you and offer an initial appointment.
Our medical office assistants will need to know your health care concern in order to book your appointment with the most appropriate health care provider.
At Health Point Care Centre, we work as a team in providing your care and you may see a doctor, nurse practitioner, registered nurse or licensed practical nurse on your visit. It may not be necessary for you to see the doctor each time you come.
As a patient at Health Point Care Centre, you are willing to:

participate in assessment by Health Point Care Centre team members to determine your care needs;

participate in and follow the plan of care to support your health as developed by you and the care team;

inform your Health Point Care Centre team members if you have concerns or feel uncomfortable; and

be courteous and respectful of other patients, visitors and members of your Health Point Care Centre team.
