Heart Function Clinics
Who can participate?
People living with heart failure, and their support network, who reside on Vancouver Island or surrounding communities. A doctor's referral is required.
How we help
We provide education and medical management to improve the quality of life for people living with heart failure and to help them manage their heart health to the best of their ability. We do this through:
- Contact with the clinic for about 6 months. This will vary based on needs;
- Medical assessment by a heart doctor and tests as required;
- Education and support;
- Exercise guidance as required;
- Referrals to other programs and services as required.
Atrial Fibrillation Clinic
Who can participate
People experiencing atrial fibrillation (atrial fib) , who reside on Vancouver Island or surrounding communities. A doctor's referral is required.
How we help
The goal of this provincial initiative is to improve access to care for patients living with this rhythm disorder. “Improving your health, improving your life”.
We provide education and medical management to improve the quality of life for people experiencing atrial fib, a type of heart rhythm disorder. We do this through:
- Consultation with a heart specialist doctor (cardiologist) and registered nurse or pharmacist, to establish a safe and individualized treatment plan;
- Ordering tests or procedures
- Telephone follow-up to help plan care and treatment;
- Counselling and information about atrial fibrillation and medications;
- Group education sessions about atrial fib and its treatment options;
- Referrals to other programs and services as required.
Pacemaker Clinics
Provides evaluation and ongoing follow-up for patients with implanted pacemakers and defibrillators to ensure each device is working properly and to monitor the life of the device. Pacemaker Clinic Services(PDF)
Lipid Clinics
Provides lipid (cholesterol) management by a doctor for people having difficulty managing their dyslipidemia.
