Heart Function Clinic

The goal of the clinic is to improve the quality of your daily life and to help you manage your heart health to the best of your ability by providing education and medical management through:

 Nurse Case Management

  • Provides telephone follow-up to help guide your lifestyle changes, provides education, and counseling, and help you develop the skills you need to live with heart failure.
  • Assists you to determine which additional education and exercise services are suitable to meet your needs. Attend our physician approved  supervised Take Heart* (Community or Hospital) exercise program to assist you to develop an individualized exercise program.

Heart Function Clinic

  • Meet our Heart Function Clinic Cardiologist who provides medical consultation.  An echocardiogram (ultrasound of the heart), ECG and blood work may be ordered as part of the assessment.
  • Meet our multidisciplinary team members, such as a nurse case manager, a dietitian, or another health care professional to support you or discuss ways to minimize further heart disease factors and / or ways to minimize your risk of further heart disease.

Additional Services

Program staff will help you increase your understanding of heart disease by linking you to:

  • Nutrition counseling
  • Heart to Heart*
  • A Change of Heart*
  • Chronic Disease Self Management


What is the cost?

Some services are provided free of charge, whereas other services have user fees. Services with user fee have an
asterisk*; contact  staff for information on a specific service.


Who can participate?

People living with heart failure (& their support person) who reside on Vancouver Island or surrounding communities.

How do I enter the program?

Program entry is through referral by your physician.

What should I expect?

  • Contact for approximately 6 months; the amount of contact will vary depending on your needs
  • Medical assessment and investigation as required
  • Education & support
  • Exercise guidance and prescription if appropriate
