Gathering information (PDF): This tool will help you when you need to gather information about what is happening, what needs to be changed, how people are reacting and to solve problems related to change. You can use this tool to decide what method you will use to gather information and to guide you in collecting, analyzing, providing feedback and developing action plans.
Current reality/desired future (PDF): This tool will help map out both the current reality and the desired future. (I.e. Number of FTE's, number of patients/residents/ clients, number of beds, etc.) Being able to see the connections between the two will help you pinpoint your short-term objectives and strategies and help you plan your next steps.
Enlisting others (PDF): Research has shown that people can be classified into groups based on the speed at which they adopt innovation and accept change. This tool will help you identify where different team members fall on the "change adoption continuum" and adjust your efforts accordingly.
