Adult Community Support Services (ACSS) provides community based case management services to support adults (clients) with severe and persistent mental illness such as Schizophrenia, Bipolar illness and major Depression.
Clients are supported to live independent and self-sufficient lives in the community and receive support and/or treatment in their own environment that is appropriate to their needs.
Care plans are developed in collaboration with the client. If a client is incapable of participating in the planning of his or her treatment, then family, friends and/or other relevant professionals are consulted.
- Supportive counselling and psycho-education to individuals, groups, and families
- Psychiatric consultation and review
- Access to housing resources and to psycho-social rehabilitation programs
- Connection with community resources and navigation of healthcare and social systems.
