713 Outreach is an integrated and multi-disciplinary team made of a blend of staff from AIDS Vancouver Island and Island Health. We are client-centered, strengths-based and use a harm reduction approach. Our service is for individuals whose needs have not been adequately met by existing case-management services. Applicants will be engaged in self-identified high levels of substance use. The majority of our work will be done in the community and often in a variety of downtown service provider’s locations.
To be eligible the applicant must be engaged in a self-identified high level of substance use. The following criteria will also be considered:
- Individuals who are 19 and older; youth who otherwise fit the criteria will be considered.
- Challenges associated with housing, income or health.
- Difficulties accessing health and social services.
- Mental health concerns.
A referral can be made through Pathways or through individuals in the community. To make a community referral, please contact our Info line for specific information and a referral form.
