Access Services

• This program receives referrals from family physicians or nurse practitioners.
• Please fax a completed physician's referral form and patient questionnaire to 250.381.3222.
• A copy of our forms can be accessed by calling our office at 250.519.3485.
• An intake clinician will review the referral and, if necessary, contact the client to conduct a further screening and assessment interview and forward the referral to the appropriate service.
