Mental Health and Substance Use Services Intake

Provides screening, assessment, treatment recommendations and referrals to Island Health Mental Health and Substance Use and external community programs in the Victoria area.  The intake team includes professionals with backgrounds in social work, nursing, substance use treatment and psychology, with psychiatrist support to make referral decisions.

Access Services

  • Mental Health Programs require a referral from family physician or nurse practitioner. Please fax a completed physician's referral form and patient questionnaire to 250.381.3222 (link to forms below). 
  • Self-referral for Substance Use Services is initiated by attending in person at Substance Use Intake at 1125 Pembroke Street, between 9:00 am and 8:00 pm daily and arranging for an appointment to discuss service options.
  • Referrals from Island Health services can be sent online via Pathways.  Clients referred this way will also be required to attend in person, please phone the Intake team to alert them to the Pathways referral.

Physicians can consult with a Mental Health Intake worker by calling 250-519-3485, or a Substance Use Intake Worker by calling 250-213-4444.

The referral will be processed once Intake has received both the Referral Form and the Client/Patient Questionnaire.
