This clinic provides cognitive and mental health assessment and limited follow-up for adults age 75 and older with late-onset mental health problems.  Services are provided in an outpatient clinic setting.

This service requires a referral from your family doctor.  The Older Adult Mental Health & Addiction Services (OAMHAS) Referral Form can be faxed to OAMHAS Intake at 250.356.9342.
