Coming to the Clinic

This page provides information about admission to the Pain Program for care providers and those in need of care.

Admission Process

To receive treatment at either of our clinical centres, please contact your family physician for a referral.

Evaluation in Pain Management

At the first visit to a VIHA Pain Clinic, a thorough assessment takes place, involving several disciplines.

The assessment includes a review of previous medical records, a pre-visit questionnaire, and an initial examination. The pain physician and other health care professionals assess the patient.

Recommendations for treatment are made and an initial treatment may be done during the first visit. 

Treatment options may include:

  • prescriptions
  • procedures
  • occupational therapy
  • psychology
  • physiotherapy
  • psychiatry  

The patient will also be given an appointment to return for a medical follow-up visit, if needed. 

Pre-Visit Questionnaire

Before your first visit to the clinic, we ask that you fill out a Patient Questionnaire (Download and print Patient Pre Questionnaire (Word) or download and print Patient Pre Questionnaire (PDF), and bring it with you when you come to the clinic.

This will help us determine what pain management techniques might be best suited for you and your lifestyle.
