Referral Process
To refer a patient to the clinic, please print and complete the referral form (download in Word format or download in PDF format).
Please mail or fax the completed form to the appropriate location (RJH or NRGH). When possible please use inter-hospital mail.

Complex Case Rounds
Complex case rounds are held weekly at the Pain Clinic and serve as an opportunity for each member of the interdisciplinary team to assess a patient.
We also invite the patient’s family physician, physiotherapist, and other healthcare providers to contribute to the process.

Clinical Practice Guidelines
The Canadian Medical Association has been active in the promotion of clinical practice guidelines (CPGs) for several years. They maintain a database of guidelines developed or endorsed by Canadian national or provincial health organizations or expert groups. This database, named the CMA Infobase, is available on the Internet at: http://cma.ca/infobase. Since its inception in 2000, it has grown to be a highly regarded and highly valued clinical electronic resource on the cma.ca web site by the CMA physician membership.
