Welcome to Primary Health Care, Chronic Disease Management, and Rural Health Services
As a program within the Vancouver Island Health Authority (VIHA), we focus on three key areas:
Primary Health Care (PHC) includes services provided by family physicians, nurse practitioners, health information lines, pharmacists, and physiotherapists.
Chronic Disease Management (CDM) includes support to help you manage the symptoms of diseases such as: depression, diabetes, cardiovascular risk, heart failure, and kidney disease.
Rural Health Services (RHS) include a broad range of health services to help protect and promote the health of individuals in rural and remote communities.
