VIHA manages four Primary Health Care Organizations. They are a point of contact between a patient and the health care system. They offer patient access to a team of care providers and are designed to meet the particular needs of communities.

An Interdisciplinary Approach to Care

The Primary Health Care Organization model uses an interdisciplinary approach to care and provides the first point of contact between a patient and the health care system. It focuses on ensuring patients meet with the right care provider at the right time, and that health services are available for extended hours.

Elements of a Primary Health Care Organization

The Primary Health Care Organization model provides patients with a first point of care complete with a full range of primary health care services including: health promotion, disease prevention, periodic care of more acute illnesses, ongoing management for those with chronic diseases, health education and promotion, counselling, and palliative care.

    • Extended clinic hours and on-call service.
