Health Point Care Centre is a multidisciplinary team of health professionals that provides primary health care services to adults over the age of 55, who are living within a defined geographic area (east of Douglas Street and south of McKenzie Avenue), who do not currently have a family doctor.
Health Point Care Centre's multidisciplinary team provides:
- acute illness care
- disease and injury prevention
- health promotion, education and counseling
- chronic disease management
- geriatric assessment and care
- palliative care and support

Health Point Care Centre is just one program housed at the
Hillside Seniors Health Centre, which offers integrated services to adults over the age of 55 living in the community and their family caregivers who reside in Greater Victoria.
