A multidisciplinary assessment and rehabilitation program aimed at keeping seniors healthy, on their feet and in their homes.
Neurological Rehabilitation Outpatient Program
Provides short-term rehab services to adults who have acquired significant neurological impairments that impact their ability to function. Therapy services available include Occupational Therapy and Physical Therapy. Education, monitoring & support re management of diabetes, self testing, diet counseling. Each Tuesday morning by appointment. Drop in most Tuesdays at 2 pm – 3 pm. 1st Friday morning of the month support group. Health practitioner referral

Ladysmith Family Practice Clinic
Provision of team based primary health care – acute illness & injury care, chronic disease management, palliative care, assessment & referral, preventative and wellness. Physicians, Nurse Practitioners, RN’s, Support staff. Hours: Monday to Friday – variable – by appointment. Referral: Self, priority given to patients who do not have a local practitioner.
Child Health Clinics
Infant and childhood immunizations and information or referrals for healthy growth and development. Clinics run on Thursday from 0900-1500 by appointment only.
Youth Health Clinics
Drop in clinic for youth up to age 25. Services include birth control , pregnancy testing, STI testing and counselling. Staffed by Public Health Nurses and Nurse Practitioners. Hours: Wednesdays 2:00 pm – 4:00 pm.

Child and Youth Mental Health Services, MCFD
Multidisciplinary Team providing clinical and psychiatric assessment, treatment, integrated case management, consultation and advocacy to children, youth and their families who are seeking assistance with a Mental Health concern. The Aboriginal Outreach team provides Mental Health Services to urban Aboriginal, First Nations and Metis children and their families. Nanaimo Main Office Hours: 8:30 am - 4:30 pm. Referral: Open to anyone to call Intake Clinician through Nanaimo.
Youth & Family Addiction Services
Individual (less than 19 years old) and/or family counseling related to substance use/misuse. Hours: 8:30 am – 5:00 pm Mon. – Fri. Anyone can refer, provided the youth has consented to the referral, as services are voluntary.
Adult Mental Health & Addictions
Intake assessments, screening, short-term follow up & referrals to specialized mental health services in Nanaimo and Duncan and/or community resources for those 19 years old and up with mental health & addictions issues. Hours: Monday to Friday by appointment. Referral: Self-referrals, clinicians, GP’s, community agencies.

Long Term Care Case Manager – assessment of need for assistance in the home, referral to other HCC services & Adult Day Care or admission to Assisted Living or Residential care. Referral: Self, family or practitioner/clinician by calling Intake in Nanaimo.
Home Care Nursing – nursing care in the community – dressings, teaching, palliative care and assessment and referral to other clinicians. Referral: Self, family or practitioner/clinician by calling Intake in Nanaimo.
Home Support - Assistance (e.g., bathing) for eligible clients who have health care needs as a supplement to support self/family care to allow the client to remain in their home. Referral: Home & Community Care clinician
Therapeutic Services - Home assessment, education and treatment. Stroke Rehab program. Staff: Occupational Therapist, Physiotherapist. Referral: Self, family or practitioner/clinician by calling Intake in Nanaimo.
Adult Day Services
Day program for eligible adults – mostly seniors – to provide social, physical and mind stimulating activities as well as respite for caregivers. Includes education, treatment and assessment by clinicians as appropriate. Transportation available. Hours: Monday – Friday as per individual assessed need. Seniors' Outreach Team (SORT)
Outreach clinical assessment and support services for those 65+ who have concerns about their mental, emotional and physical well-being and/or use of medications or alcohol. Includes psychiatric consult, nursing and occupational therapy assessment, education and follow up care. Referral by self or practitioner/clinician to intake
Non invasive radiological exams. Hours: Mon – Fri: 9:15am - 12:15pm, 1:30pm - 4:15pm and 5:00pm - 6:00pm. Referral: Health Care Practitioner.
Specimen collection, ECG’s, 24 hour heart monitors ("Holters"). Hours: Monday – Friday: 7:00 am – 4:00 pm, Saturday: 8:00 am – 11:00 am. Referral: Health Care Practitioner by requisition.
