The Ladysmith Community Health Centre, formerly the Ladysmith Hospital, is a Primary Health Care Centre designed to provide a range of health and community services in one central location. Planning for this primary health care site began in 2003 and included a review of the health services provided to the Ladysmith community. The results of this planning included the health centre - services outlined below - and a privately run 101 bed residential facility built adjacent to the Centre. The goal of the Centre is to provide integrated and collaborative team care to patients by the appropriate provider in the appropriate setting in an appropriate time and in an appropriate type and format of intervention for their needs. Neurological Rehabilitation Outpatient Program
Provides short-term rehab services to adults who have acquired significant neurological impairments that impact their ability to function. Therapy services available include Occupational Therapy and Physical Therapy. Diabetes Education Education, monitoring & support re management of diabetes, self testing, diet counseling. Each Tuesday morning by appointment. Child and Youth Mental Health Services, MCFD Multidisciplinary Team providing clinical and psychiatric assessment, treatment, integrated case management, consultation and advocacy to children, youth and their families who are seeking assistance with a Mental Health concern. Home Support - Assistance (eg. bathing) to eligible clients who have health care needs as a supplement to support self/family care to allow the client to remain in their home. Referral: Home & Community Care clinician

Adult Day Program Day program for eligible adults – mostly seniors – to provide social, physical and mind stimulating activities as well as respite for caregivers. Includes education, treatment and assessment by clinicians as appropriate. Transportation available. Hours: Monday – Friday as per individual assessed need. - Seniors' Outreach Team (SORT)
Outreach clinical assessment and support services for those 65+ who have concerns about their mental, emotional and physical well-being and/or use of medications or alcohol. Includes psychiatric consult, nursing and occupational therapy assessment, education and follow up care. Referral by self or practitioner/clinician to intake 250-739-5777
