Home and Community Care Dietitians

On Southern Vancouver Island, Home and Community Care Dietitians provide in-home nutrition care to homebound clients at nutrition risk.  Registered dietitians provide assessment, consultation, and education to support clients to optimize their nutritional health while continuing to live at home.

Home and community care dietitians can help address health issues like:

  • swallowing disorders,
  • malnutrition,
  • healing of wounds and pressure ulcers,
  • tube feeds,
  • gastro-intestinal diseases,
  • cancer,
  • kidney disease, and
  • diabetes.

Across Vancouver Island, Health Services for Community Living Dietitians provide nutrition care to support adults with a developmental disability to maximize their health, independence and participation in the community.  Information and education assists people with developmental disabilities, their caregivers, and their families to respond to specific health care needs.
