1. VIHA Service Delivery Frameworks
Service frameworks are an approach to integrated health planning. This project will develop service delivery frameworks for VIHA’s chronic disease management priorities. One focus of service delivery frameworks is to identify gaps in care and make recommendations for change. Another is to follow the patient’s journey through the health care system over the course of an illness.
The service frameworks provide an opportunity to recommend service delivery redesign options to improve patient outcomes, facilitate quality care and promote optimal use of heath services within VIHA’s geographic boundaries.
VIHA’s priorities for service delivery frameworks over the next three years are:
- Year 1: Diabetes, congestive heart failure and depression
- Year 2: Hypertension, asthma, and lung disease
- Year 3: Liver disease, osteoarthritis and rheumatoid arthritis
2. Primary Mental Health Task Force
A VIHA survey conducted in 2005/06 found that 77% of physicians identified people living with mental health and addictions conditions as those that could best be served by better integration and coordination between services.
VIHA’s response is to create a Primary Mental Health Care Task Force and network of health providers and other key partners inside and outside the health authority. The aim of this investment is to determine priorities for improvement and to gain common understanding of the issues.
Key objectives for the network are to continue:
- professional education programs
- development of best practices
- sharing resources and knowledge gained through quality improvement initiatives.
3. VIHA Chronic Disease Management Steering Committee Secretariat
Serves as secretariat to the Chronic Disease Management Plan Implementation Steering Committee, whose mandate is active until March 31, 2009.
The mandate of the Committee is to:
- Oversee implementation of the priority initiatives identified in the VIHA CDM Plan 2006/07-2008/09.
- Lead the direction, planning and implementation of integrated CDM programs within VIHA through development of collaborative partnerships.
- Provide a forum for communication with key partners.
- Serve in an advocacy capacity with internal and external partners in the creation of healthy public CDM policy.
