Providing the highest quality patient care and safety in Island Health is one of our top priorities. Island Health has a variety of strategies, initiatives and tools to support excellence, including the clinical care management (CCM) initiative.
CCM is a province-wide initiative. B.C.’s six health authorities, Ministry of Health and BC Patient Safety and Quality Council have identified guidelines for specific clinical areas where the evidence shows that compliance to the guidelines can make a significant difference in the quality of patient care.
Under this initiative, Island Health staff review specific patient care activities to determine if the guidelines are in place. Staff collect data on the identified patient care practices and report to the Ministry of Health on a quarterly basis. The information collected through CCM allows us to provide feedback to staff in the clinical areas on progress to guideline implementation. Because some of the tools and processes for collecting data under CCM are relatively new, the data available for each clinical area currently vary.
