Island Health provides quality care for thousands of Vancouver Island residents every year. The complex and diverse nature of health care means there is always some risk involved in the delivery of this care. At times, things can and do go wrong.
The Canadian Adverse Events Study (Baker et al., 2004) found that an estimated 7.5% of patients admitted to acute care hospitals in Canada experienced at least one adverse event during their stay.
Our staff, physicians and leaders examine patient safety events so we can learn and improve from them, and prevent recurrence. Analysis of patient safety events (including near misses) is conducted in two ways: prospectively (preventive action before an event occurs); and retrospectively (following a particular patient safety event).
