Reporting Adverse Events and Near Misses

for staff and physicians

The Patient Safety & Learning System (PSLS) is a web-based tool used by healthcare providers to report and learn from patient safety concerns such as actual adverse events, good catches (near misses) and hazards.

PSLS helps healthcare providers, leaders, and others to collect and analyze information that is crucial for patient safety and quality improvement. 

Island Health first implemented PSLS at Saanich Peninsula Hospital in February 2010 with the full implementation completed at the end of 2011. PSLS is in use across all BC health authorities.

The Patient Safety and Learning System is available to Island Health staff via the intranet.*
